View productIf you have tried iron supplements and stopped because of the nausea, constipation or stomach cramps, you are not alone. GI side effects are the most common reason women discontinue synthetic iron. Haem iron from beef liver absorbs through a completely different pathway, one that is gentler on the gut and largely unaffected by the dietary inhibitors that complicate non-haem absorption. That does not make it a universal solution, but for women who need iron support without the GI trade-off, it is worth understanding why it works differently.
Haem iron is taken up by intestinal cells through a different route from free non-haem iron, and is less affected by dietary inhibitors (Hurrell and Egli, 2010). Haem-iron supplements are often reported as easier to tolerate than ferrous sulphate, though the comparison has not been settled in large head-to-head trials (Kalman, 2025). Ferrous sulphate, the most commonly prescribed form, releases reactive iron in the gut lumen, which is what causes the nausea, cramping and constipation many women experience. Haem iron is absorbed as an intact porphyrin complex, avoiding most of that disruption. If your issue is tolerability rather than a need for a high therapeutic dose, beef liver offers a gentler route to maintaining iron stores.
Ferrous sulphate and ferrous fumarate release free ferrous ions in the stomach and small intestine. These ions are reactive and generate oxidative stress in the gut lining, which triggers the nausea, epigastric pain and constipation that are well documented with supplemental iron. The problem is dose-dependent: higher doses mean more free ions, more oxidative irritation, more side effects. Enteric-coated and slow-release formulations reduce but do not eliminate this, and they also reduce the amount of iron absorbed.
Haem iron does not release free ions. It enters the enterocyte as an intact iron-porphyrin complex via its own carrier protein, where the iron is released intracellularly. The gut lumen never sees the reactive free iron that causes the trouble.
Standard synthetic iron (ferrous sulphate) delivers a high dose at the cost of GI side effects. Gentle iron formulas, typically iron bisglycinate, chelate the iron to reduce gut irritation and do improve tolerability, but they are still non-haem and still compete with phytates, tannins and calcium for absorption. Their absorption rate sits in the non-haem range of 2 to 20 percent depending on the meal and formulation.
Haem iron from beef liver absorbs through a separate pathway entirely, at roughly 15 to 35 percent, and is largely independent of dietary inhibitors. So it is both gentler and better absorbed. Where synthetic iron wins: when a doctor has prescribed a specific high dose for confirmed deficiency and needs to raise levels quickly. Beef liver cannot match the concentrated dose of a prescription tablet, and is not designed to. It is ongoing whole food support, not acute intervention.
